    Project Charter Information

    The project charter establishes the basic plan and content that guides how we manage Chronic Obstructive Pulmonary Disease (COPD) patients. The charter explains the project aims to cut COPD hospital returns and enhance patient care using proven medical methods (Mammen & Sethi, 2020). The charter specifies what individual team members must do while defining when tasks must be completed and how success should be measured. Stakeholders agree on this document to ensure everyone understands what they need to do to achieve project objectives.     Gap Analysis

    We analyzed how existing COPD treatment methods fall short of ideal standards to show where we need to make improvements. We examined the differences between current and optimal health processes to identify issues in follow-up care and patient information along with teamwork problems. We used a gap analysis tool to discover clear steps that would fix our patient care problems by linking medical teams better and getting patients more involved while using healthcare resources properly. The study results proved that our project needed to move forward since it would help bring consistent care and lower the risk of COPD patients needing to be readmitted to the hospital (Yawn et al., 2021).

    Evidence to Support the Need

    Multiple peer-reviewed papers connected with national health statistics show support for this proposal. The Centers for Disease Control tracks that COPD drives the majority of hospital admissions and returns to hospitals at rates above 20% during the first month after discharge. Research proves that teams of healthcare providers can reduce repeated hospital visits by 40% or better (Turner & Kelle, 2022). Research shows that telehealth technologies and personalized patient training help COPD patients take their medicine better while managing their health and living better with the disease. The research shows that we need to set up organized COPD treatment programs to solve these medical recovery problems.

    PICOT

    The PICOT framework guides the project’s evidence-based approach:

    • P (Population): Adult COPD patients who have a higher chance of returning to the hospital.
    • I (Intervention): Our team will introduce an inclusive health model that teaches patients, uses remote medical checks, and connects care providers across teams.
    • C (Comparison): The standard treatment methods that do not include extra education and coordination services.
    • O (Outcome): The healthcare team works to lower patients’ return visits to hospitals while helping them feel better in their lives and experience better care.
    • T (Time): We track results for six months after introducing the new approach.

    We use the PICOT structure to base our intervention plan on research evidence while setting specific metrics for performance evaluation.

    Stakeholders

    The project needs multiple stakeholders to provide their professional skills and necessary support. Key stakeholders include:

    • Healthcare Providers: Healthcare providers will manage patient treatment while putting these project strategies into action.
    • Patients and Caregivers: Strong patient involvement in learning and taking care of themselves helps the project reach its goals.
    • Hospital Administration: Superior staff support directs project funding and matches initiative objectives with policy requirements to maintain ongoing success.
    • External Stakeholders: Our project gains additional reach and funding when health agencies and insurance companies participate (Thomas et al., 2021). We will achieve stakeholder participation by scheduling ongoing meetings to create shared decisions with full transparency and clear communication.

    Team Leader

    The team leader guides project performance by supervising tasks and making sure everyone works toward desired goals (Smith & Jacobs, 2021). The leader needs to guide the team well with good management habits know how to handle patients with COPD and work with different healthcare professionals together. The team leader connects stakeholders and oversees project communication to solve problems while keeping the project moving forward. They will direct data experimentation and research to validate if project results match planned targets.
